Registration Information & Forms

This page provides details on how to register to play Hounds Ice Hockey. Prior to your first practice, you will need to do the following:

  1. Complete a BAHHA Registration Form for the Current season
  2. Complete a USA Hockey Consent to Treat Form
  3. Complete a USA Hockey Code of Conduct Form
  4. Make a copy of the player's Birth Certificate to provide to BAHHA (if the player has not played on a Hounds team in the past)
  5. Go to USA Hockey Registration page and follow ALL THE instructions on that page.

Please provide to BAHHA the 3 completed forms (and the player's birth certificate if necessary) by

either mailing these to:

OR

Put the forms in an envelope and drop off in the BAHHA drop-box located at Space City Ice Station. Our drop-box is a lockable, black mailbox with a BAHHA logo and it is hanging on a pillar near the skate rental area.

Shortly after you have registered with USA Hockey and provided us with that information (per instructions on the USA Hockey Registration Confirmation page), your name will appear on the USA Hockey Registered Players List 2008-2009 (the list will be updated as confirmation is received) - Note that USA Hockey memberships are good for 1 year and must be renewed every Fall.

If your player is playing Travel Hockey, please read the Travel Hockey Info Letter. This gives you info on practices/games & costs.
